Job Description Licensed Practical Nurse Job Title: LPN 1 Department: Resident Services Reports to: Director, Resident Services Classification: PC8 Wage Level: Per NBA Collective Agreement Wage Schedule Date Reviewed: May 2018 Job Summary: Reporting to the Director, Resident Services, under the clinical supervision of the RN/RPN Supervisor, and in accordance with the LPN scope of practice and standards and Kiwanis Pavilion policies and procedures, the LPN provides safe, competent and ethical nursing services, provides clinical guidance to health care workers on assigned units, and collaborates with health-care team members and residents families/representatives to achieve high quality resident care outcomes. List of Duties: Performs comprehensive health assessments of residents to identify problems, strengths and needs. Applies critical thinking to respond to and report assessment findings. Contributes to the development and modification of resident care plans based on assessment findings and desired outcomes. Attends and provides input into multidisciplinary care conferences, family meetings and quality improvement initiatives. Performs nursing care and procedures and evaluates outcomes. Processes physicians orders and administers medications to residents according to policies and procedures. Assists with provision of personal care, meals, and transportation to social activities to as required. Participates in admission, discharge, and transfer of residents according to policies and procedures. Documents pertinent information and observations regarding resident status, care provided and outcomes. Reports problems and/or changes to designated staff and residents representatives as required. Completes incident reports as required. Performs other related duties as assigned. Qualifications, Skills and Abilities: Graduation from a recognized Practical Nursing program, or equivalent combination of education and experience. Current Practising license with CLPNBC. Ability to communicate effectively both verbally and in writing. Ability to initiate, maintain and terminate collaborative relationships with residents, families, and health-care team members. Mental and physical ability to carry out the duties of the position. Ability to plan, organize and prioritize work. Ability to operate relevant equipment/technology.
