LOCATION: Yale First Nation SALARY RANGE: as per BCNU grid (please note this is not a union position, 5% in lieu of benefits added) START DATE: as soon as possible
POSITION SUMMARY The Community Wellness Nurse (CWN), in collaboration with the health director and the community will contribute to the overall health and wellness of the community using a population health and community development framework. The CWN promotes traditional and cultural approaches to health practices.
The CWN possesses an understanding of the determinants of health and demonstrates values of community health promotion, commitment to creation of health partnerships, and community development strategies.
The CWN works in partnership with community and staff members to deliver programming and care in a culturally safe and respectful manner.
Respectful engagement with community members as equal partners in health service provision includes active engagement with clients in homes and community to promote mental, spiritual, emotional, and physical wellness.
The CWN will also be responsible for the administration Jordans Principle education and service access applications and supports.
QUALIFICATIONS AND VALUES Active membership in the College of Licensed Practical Nurses of BC - experience an asset but not required Current CPR-C certification Current immunizations Unrestricted BC drivers license Computer literacy Foot care certification or willingness to complete Familiarity with Jordans Principle and willingness to educate self and community on benefits/supports Lifelong learner with an interest in furthering education through programs and workshops Candidates with First Nations ancestry will be given priority Food safe and WHMIS or willingness to complete Approval for Criminal Record check for Vulnerable Populations Ability to work independently and interdependently
DUTIES Assist Health Director in management and delivery of health and wellness services and programs Take a leadership role in the implementation of Jordans Principle. Excellent interpersonal skills and the ability to establish and maintain clear therapeutic relationships with community members with a client centered focus on health and wellness strategies Participation in meetings, committees, and planning sessions Organization of and participation in community events, programming and services Provision of home care nursing services in community, including but not limited to footcare, medical transportation, home support, personal care. Assessment and treatment skills including good knowledge of diabetes, arthritis, wound care, and chronic disease management involving multiple conditions Ability to initiate and maintain professional relationships with all providers who can contribute to the health and wellbeing of community members and may include coordinated assessment processes, referral protocols, discharge planning and service links with such providers as hospitals, physicians, respite and therapeutic services Ability to plan, implement and evaluate programs related to the health needs of community members using resources from many sources including the management of the Community Health Plan and the Comprehensive Community Plan. Knowledge of how to access specialized medical equipment, supplies and specialized pharmaceuticals to provide home care Ability to develop and maintain a system of standardized record keeping and data collection to carry out program monitoring, ongoing planning, reporting and evaluation activities. Engages in ongoing acquisition of evidence-based knowledge and skills necessary to maintain competent nursing practice Abides by ethical standards of confidentiality and College of Licensed Practical Nurses Code of Ethics
COMPETENCIES BODY OF KNOWLEDGE Health and Mental wellness promotion rooted in a well-informed knowledge base of the socio-historical context of First Nations including residential school trauma, and ongoing socio-economic and educational inequities Promotes community and individual mental, physical, emotional and spiritual wellness across the lifespan Works towards illness prevention through screening and early identification, mental health and personal health assessments and monitoring well-being Demonstrates an understanding of self-care concepts Encourages client/family independence when appropriate and can teach client and family using principals of adult learning Sets priorities when planning and delivering care Assesses needs/strengths/resources, plans, implements, and evaluates programming regarding healthy lifestyle, early detection of illness, emotional wellness, womans health topics, mens health topics, child and youth health topics, injury/ fall prevention, and elder abuse and neglect prevention. Demonstrates ability to manage caseload Adapts nursing skills to home environment, client need and cultural expectations Demonstrates ability to collect, interpret and apply meaningful data for client care using standardized charting records Recognizes when supervisor should be informed that care plan should be changed due to change in client status Participates in community Quality Assurance and Quality Improvement activities Demonstrates a respect of clients rights within the culture of the community demonstrates qualities of honesty, integrity and respect Seeks assistance when needed
