Licensed Practical Nurse 
LPN Job Summary The Licensed Practical Nurse (LPN) assesses, plans, implements, and evaluates nursing care for their work assignment in accordance with competency guidelines within the Professional Standards outlined by the College of Licensed Practical Nurses of BC (CLPNBC), current legislative scope of practice and in accordance with the established policies and procedures of Pro Vita. Responsibilities and Duties Responds to Care At Home Services staff regarding shift/patient requests via email or phone in a timely manner. Assesses and monitors residents/patient s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Where appropriate encourages residents/patients participation in decision making in regards to their plan of care and activities of daily while maintaining independence as much as possible. Where appropriate encourages residents/patients participation in providing their own personal care. Encouraging them to be as independent as possible to towards achieving an improvement in their care and assistance needs. Contributes to the development and modification of the individualized plan of care, by providing input into interdisciplinary care conferences, medication reviews and other meetings as required. Under the guidance of the DOC or Direct Nursing Supervisor and in accordance with the established policies and procedures, administers prescribed medications and treatments. Performs nursing care and procedures and evaluates outcomes; records observations and reports problems and/or changes to the Nurse Case Manager; Pro Vita Manager,Registered Nurse or Direct Nursing Supervisor. When required performs admissions, discharges, and transfers of residents/patients according to established Pro Vita policies and procedures. Observes, monitors, documents and updates information about resident s physical and psychological condition, care, and nursing procedures. Supervises and provides direction to the nursing care team including reassignment of workload. Provides and assists with personal care to residents/patients as required. Assists residents/patients with meals, ensuring they receive adequate nutrition according to individual care plans and reporting changes in appetite/food intake to Registered Nurse and/or Dietician. Completes Serious Incident Reports as required. Participates in staff education and provides orientation of students and new personnel to job requirements, equipment and routines. Assumes responsibility for own continuing education and professional development; maintains current knowledge of trends and concepts in gerontological nursing. Promotes and maintains positive communication with all residents/patients , staff and the public. All safety rules and safe work procedures are adhered to at all times. Reports all hazards, incidents, near misses and workplace injuries to the Nurse Case Manager or Nursing Supervisor immediately and in writing. Does not participate in unsafe situations (horseplay), and corrects any unsafe conditions if able. Actively participates in all Health and Safety initiatives and make suggestions to improve Workplace Health and Safety. Performs other related duties as assigned. Qualifications Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training, and experience recognized by the CLPNBC. Maintains Practicing Full Scope Licensure with the College of Licensed Practical Nurses of British Columbia (CLPNBC). Maintains Valid First Aid and CPR A . Must maintain Occupational First Aide Level 1 status (may be a Level 2 depending on site) MDS education (Gold Care or Point Click Care) Meets the physical requirements for the role. 1-2 years experience in a long term care facility is preferred. Listening: Practices attentive and active listening; has the patience to hear people out; can accurately restate the opinions of others even when he/she disagrees. Problem Solving: Uses rigorous logic and methods to solve difficult problems with effective solutions; probes all fruitful sources for answers; can see hidden problems; is excellent at honest analysis; looks beyond the obvious and doesn't stop at the first answers. Time Management: Uses his/her time effectively and efficiently; values time; concentrates his/her efforts on the more important priorities; gets more done in less time than others.
